A current/valid Certificate of Insurance is needed.

To ease this process, we suggest the following 3 steps be taken:

• Fax this checklist to your Insurance Co. - Ensure your name is provided below.
• Follow-up with your Insurance Co. to ensure that you have proper coverage (see checklist below)
• Have your Insurance Co. provide a Certificate of Insurance to us that indicates the required coverage.

Reel Men requires the following coverage shown on the Certificate of Insurance:

	General Liability (Typically $1M)
	Hired Auto Liability (even if truck is not being rented. This covers liability that General Liability does not cover, e.g. injuries sustained while loading/unloading heavy equip such as Fisher dollies.)
	If a grip truck is rented, we also need Hired Auto Physical Damage (Value = $35,000.00)
	Misc. Rented Equipment (may also be stated as: Prodn Pkg or Producers Pkg or Entertainment Pkg or Equip Floater or BPP (Business Personal Property). If BPP, certificate must indicate coverage at all unnamed locations and intransit) - Anything else will not cover our equipment. Notes: Coverage should indicate replacement cost and must include the value of all rented equipment. (Please call us if this value needs to be determined.) If our 10 Ton Tractor/Trailer (with mounted generator) is rented, the certificate needs to state "Misc Rented Equip includes truck mounted generator."
	Reel Men must be named as Additional Insured (General & Auto Liability).
	Reel Men must be named as Loss Payee (rented equipment and vehicles provided).
	If Rob is hired to work the job, please also indicate Workers Comp coverage.

IMPORTANT

If the certificate holder is an ADDITIONAL INSURED, the policy(s) must be endorsed. A Statement on this certificate does not confer rights to the certificate holder in lieu of such endorsement(s). If SUBROGATION IS WAIVED, subject to the terms and conditions of the policy, certain policies may require an endorsement. A statement on this certificate does not confer rights to the certificate holder in lieu of such endorsement(s).
